If you are enrolling dependents, please include copies of social security cards for each dependent, a copy of a birth certificate for each child, and a copy of your marriage license if you are married and are enrolling your spouse. If you are enrolling a registered domestic partner, please submit a copy of the State Certification or a copy of LACCD's Affidavit of Domestic Partnership (and if your domestic partner is your tax dependent, please submit the Affidavit of Domestic Partner Status and Tax Dependency Status.)
